How Does Janumet XR Work?

As mentioned, Janumet XR contains two diabetes medicines: sitagliptin and metformin ER. These two medicines work in different ways to control blood sugar.
Sitagliptin belongs to a group of medicines known as dipeptidyl peptidase-4 (DPP-4) inhibitors. DPP-4 is an enzyme that breaks down certain hormones known as incretin hormones. These cause insulin to be released from the pancreas in response to meals. They also reduce the amount of glucagon released by the pancreas, which reduces glucose (sugar) production by the liver.
By blocking the DPP-4 enzyme, Janumet XR increases the level of incretin hormones in the blood. This causes more insulin to be made in response to meals and less glucose to be made by the liver.
Metformin belongs to a group of medicines known as biguanides. It lowers blood sugar levels in several ways, such as:
  • Decreasing the amount of glucose made by the liver
  • Decreasing the amount of sugar absorbed into the body through the small intestines
  • Improving insulin sensitivity, which helps increase glucose uptake and use by cells.
Janumet XR contains the extended-release version of metformin. This drug stays in the body longer than regular metformin, so it only needs to be taken once a day.

When and How to Take It

Some general considerations to keep in mind during treatment with Janumet XR include the following:
  • Janumet XR comes in the form of a tablet. It is usually taken by mouth once a day.
  • You should take your dose with a meal to minimize the risk for stomach upset. It is best to take it with your evening meal.
  • If your dose requires you to take two Janumet XR tablets, take them together once a day. 
  • Janumet XR tablets must be swallowed whole. Do not split, crush, cut, or chew them.
  • Try to take your dose at about the same time each day to keep an even level of the drug in your bloodstream.
  • Make sure to continue your diabetes diet and exercise program during Janumet XR treatment. Diet and exercise are an important part of controlling your blood sugar.
  • For the medication to work properly, it must be taken as prescribed. Do not stop taking it or take a different dose without your healthcare provider's approval.
