Janumet XR Dosage

The initial dosage of Janumet XR will depend on how well your blood sugar levels are controlled, other medications you are taking, and other factors. Your healthcare provider may slowly increase the amount to a maximum of 100 mg/2000 mg. This medication comes as a tablet that is taken once daily. Make sure to take it with food to avoid stomach upset.

 

An Introduction to Dosing With Janumet XR

The dose of Janumet® XR (sitagliptin and metformin extended-release) your healthcare provider recommends will vary, depending on a number of factors, including:
 
  • How well your blood sugar is controlled
  • Other medications you are taking
  • Other medical conditions you may have.
 
As is always the case, do not adjust your dosage unless your healthcare provider specifically tells you to do so.
 

What Is the Recommended Janumet XR Dosage?

Janumet XR contains a combination of two diabetes medicines: sitagliptin (Januvia®) and metformin ER (Glucophage XR®, Glutametz®, and Fortamet®). The recommended initial Janumet XR dose for treating type 2 diabetes can vary from person to person, but is generally based on your current dose of sitagliptin or metformin, as follows:
 
  • If you do not already take metformin, the recommended starting dose is Janumet XR 100 mg/1000 mg
     
  • If you already take metformin, the recommended starting amount is one that provides 100 mg of sitagliptin in combination with your current metformin dosage
     
  • If you currently take regular Janumet (sitagliptin and metformin), it is recommended that you start Janumet XR at the same total daily dose, but take it once a day.
 
Your healthcare provider will choose the most appropriate initial amount based on your unique situation and then slowly increase it if necessary to a maximum of 100 mg/2000 mg. Doses are increased slowly to help minimize the risk for stomach upset that can occur initially with metformin.
 
Dosage adjustments are based on how well you tolerate Janumet XR and how well your blood sugar is controlled. Your healthcare provider will check your blood sugar control with regular blood tests, including blood sugar checks you can do at home, and hemoglobin A1C blood tests.
