Janumet XR and Breastfeeding

Although Janumet XR (sitagliptin and metformin extended-release) has not been studied in breastfeeding women, it is believed that both of the active ingredients in the drug would pass through human breast milk to some extent. The manufacturer of the drug recommends that women who are breastfeeding should use this combination product with caution.

Can Breastfeeding Women Take Janumet XR?

Janumet® XR (sitagliptin and metformin extended-release) is a prescription medication used to treat adults with type 2 diabetes who need to take both sitagliptin (Januvia®) and metformin ER (Glucophage XR®, Glutametz®, and Fortamet®). It is not known if Janumet XR passes through breast milk in humans.
The manufacturer of Janumet XR recommends it be used cautiously in breastfeeding women. If you are currently nursing a child or plan to start, talk to your healthcare provider before taking this medication.

Janumet XR is a combination medication that contains two diabetes medicines. Both of the active ingredients, sitagliptin and metformin ER, pass through the breast milk of rats.
Janumet XR itself has not been studied in breastfeeding women. However, metformin has been shown to pass through the breast milk of women in small amounts. It is unknown whether sitagliptin passes through human breast milk as well. However, based on the properties of the medication, it is expected to pass through human breast milk to some extent.
Because of the potential for problems in a nursing infant, such as low blood sugar levels, caution should be used if Janumet XR is taken while breastfeeding. If your healthcare provider recommends this drug while you are nursing, watch for any potential side effects in your child, such as:
  • Diarrhea
  • Vomiting
  • Irritability
  • Sleepiness
  • Not eating well or excessive hunger.
