Fortamet Drug Information

Available by prescription only, Fortamet® (metformin hydrochloride extended-release) is a medication approved to treat type 2 diabetes (also known as noninsulin-dependent diabetes or adult-onset diabetes). Fortamet is a long-acting form of metformin. It comes in tablet form and is taken once daily with your evening meal.
 
Although the drug is generally well tolerated, it is not suitable for everyone. Before using Fortamet, make sure your healthcare provider has up-to-date information on your medical history and a list of your current medications. In particular, let your healthcare provider know if you have problems such as congestive heart failure or low vitamin B12 levels.
